Continuing Education Grant Process

To be considered for a Medication Delivery Continuing Education Grant award, please follow the application process detailed below:

STEP 1: Complete Form

A complete application packet will include the following:

  • Continuing Education Grant Request Form
  • Letter of Request from the CE Provider describing the provider's organization, history and public education mission, and detailing the educational program for which the Educational Grant funding is sought.
  • A budget detailing estimated educational program expenses that may include speaker honorarium and travel, and the percentage or dollar amount of those expenses being requested from Baxter. Please contact us if you have questions regarding items that can be covered in the budget.
  • W-9 Form

Application Forms should be downloaded and completed fully. Incomplete forms may cause delays in review and approval of the grant.

Incomplete forms will be returned to the requestor.

STEP 2: Submit Form

Upon completion, please send (1) signed copy of full application packet to:

STEP 3: Review

Continuing Education Grant requests are reviewed within one month of receipt by a multidisciplinary team within Medication Delivery Division, Baxter Healthcare Corporation, led by the Global Medical Affairs team.

STEP 4: Notification & Award

If the Continuing Education Grant is approved, the applicant will receive notification by email, along with an attached copy of the Continuing Education Agreement. The Agreement should be downloaded, reviewed, signed and returned to Baxter.


Upon return to Baxter, the request will be routed for approval through the Baxter Healthcare Fraud and Abuse Control Program. A fully executed copy will be returned to you for your records. A letter will accompany payment to the appropriate remittance address.


The process to receive funding will take about 8 weeks from the receipt of request.


If it is determined the continuing education request will not be supported, Global Medical Affairs will notify the requestor in writing of denial of funding.
